Hometown Health Center offers an array of services using the “Patient-Centered Medical Home” model.
The medical team at HHC is comprised of New York State board certified family physicians, pediatricians, internists, physician assistants, nurse practitioners, RNs, licensed practical nurses, medical assistants, case management staff and a registered nurse navigator. Hometown also offers OB/GYN, social work services and podiatry.
We deliver clinical services in a Patient-Centered Medical Home model of care. This model is a way of organizing primary care that emphasizes coordination and communication to transform care into what you want it to be. Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.
At HHC a full-time obstetrician/gynecologist provides specialty care and consultation to other providers. All family providers provide prenatal care. All family physicians and OB/GYNs have obstetrical privileges at Ellis Medicine’s Bellevue Women’s Hospital.
